Initial Direct Deposit Authorization Form
The "Initial Direct Deposit Authorization Form" below may be downloaded for your convenience. The completed form must be mailed back to the Teacher Academy with the appropriate voided check attached. This form only give initial authorization to PAGE to make a direct deposit to your account for monies owed. You MUST complete a new and appropriate expense form (found by clicking here) to receive reimbursements for expenses.

Locate the file you downloaded and double click. The form will open and allow you to enter data.
Print the completed form and mail as instructed on the form.
